Start with the question and the design

Ask what was actually compared and how. A randomised controlled trial carries more weight than a single-arm case series; a prospective study that plans its data collection is generally stronger than a retrospective chart review. Registry and real-world studies add breadth. None is perfect — the point is to match the claim to the design.

Look at the outcomes that matter

Meaningful endpoints include complication and reoperation rates, reconstruction loss, and increasingly patient-reported outcomes. Be wary of studies that report only surrogate measures or very short follow-up when the questions of interest are long-term.

Mind confounding and selection

Patients are not randomly assigned to techniques in everyday practice, so differences between groups can reflect who was chosen for what — not the intervention itself. Radiotherapy, smoking, body mass index and surgeon experience all influence outcomes and must be accounted for.

Product-specific versus category evidence

A crucial habit for the biological matrix literature: evidence for one product does not automatically transfer to another. Because materials differ by source and processing, look for data on the specific device in question.
